Common questions before booking

What is liquid FFS?
Liquid FFS is a community term for non-surgical facial feminization using dermal fillers, Botox, and sometimes PDO threads to soften masculine features without bone surgery. Results are temporary and require occasional maintenance — typically one to two visits per year to preserve natural-looking results.
How long do non-surgical facial feminization results last?
Botox results last three to four months. Hyaluronic acid fillers last 6 months to two years depending on the area and volume. PDO thread lift results last twelve to eighteen months. Most patients schedule maintenance once or twice per year depending on their goals.
Is non-surgical facial feminization painful?
Most areas are treated with topical numbing cream, and many HA fillers contain lidocaine for comfort. Patients typically describe mild pressure or a brief pinch around the mouth, eyes, or jaw. Masseter Botox injections use very small needles with minimal discomfort.
Can I get non-surgical feminization while on HRT?
Yes. Many patients use non-surgical facial feminization alongside hormone therapy. HRT provides foundational skin softening and fat redistribution; injectables refine and accelerate specific contour goals. Together they can complement each other depending on anatomy, timeline, and goals. Patients on HRT should share their full medication list with Dr.
Are Botox and fillers considered gender-affirming care?
In clinical practice, facial fillers and neurotoxins used to feminize the face are recognized as gender-affirming procedures — and a meaningful part of gender affirmation for many patients. Earlier research (Ainsworth & Spiegel, Quality of Life Research, 2010) also found significant quality-of-life improvements following facial feminization.

What Is Non-Surgical Facial Feminization?

Non-surgical facial feminization treatment zones at Desert Bloom Skincare, Scottsdale

Non-surgical facial feminization — sometimes called liquid FFS — combines minimally invasive injectables, threads, and selected device-based treatments to soften the features that most strongly influence how gender is read in the face, without bone surgery. The goal is a natural, smooth shape: softer contours, rounder cheekbones, and a more feminine overall appearance.

The approach works on three levels. Dermal fillers add feminine volume and contour to cheeks, lips, and the areas around the mouth — when hyaluronic acid is used, results can be adjusted or dissolved with hyaluronidase if needed. Botox and other neurotoxins relax overactive muscles that contribute to a more masculine appearance, reduce frown lines, and smooth dynamic wrinkles. PDO threads lift and reposition soft tissue for a refined profile. Many patients also include laser hair removal or collagen-stimulating treatments as part of a broader plan to refine skin texture and reduce unwanted facial hair.

These treatments may be appropriate for trans women, non-binary patients, and cisgender women who want softer facial proportions without surgery. They can complement hormone therapy, serve as a bridge before surgical FFS, or stand alone for patients with mild-to-moderate features. Related approaches include aesthetic facial balancing and facial sculpting with fillers. For patients exploring the opposite direction, Desert Bloom also offers non-surgical facial masculinization.

Feminizing Treatment Areas

Each feminization plan targets specific zones based on which facial features read as most masculine. Dr. Borakowski uses a combination of facial fillers, Botox, and threads depending on the area — staged so each region can be assessed and refined before moving to the next.

Common Feminization Zones

Botox + HA filler

Brow & Forehead

Lateral brow lift, frown-line softening, and camouflage for mild brow ridge. Botox to the frontalis, glabella, and corrugator pairs with HA filler for convexity. Botox lasts 3–4 months; filler 12–18 months.

HA volumizing filler

Cheeks

Restores the rounder, more feminine ogee curve. Malar and zygomatic HA placement adds midface convexity that reads as feminine. Duration 12–24 months.

Liquid rhinoplasty

Nose

Liquid rhinoplasty with HA filler or PDO nose threads can refine a wider nasal bridge or bulbous tip without surgery. Duration 12–18 months.

Filler + lip flip option

Lips

Lip augmentation for fuller body, philtrum columns, and softer border. Helps address thin upper lip and a long philtrum. Lip flip available. Duration 6–12 months.

Masseter Botox + filler

Jaw & Chin

Masseter Botox slims a wide jaw; chin and pre-jowl filler refine contour where appropriate. Specific plan reviewed at consultation. Botox 4–6 months; filler 12–18 months.

HA filler or PRP/PRF

Under Eyes

Tear-trough HA filler works well for younger patients with true volume loss. For patients 40+ or those with thinning skin, PRP/PRF is preferred. Duration varies by patient.

HA filler

Nasolabial Folds

Softens deep smile lines that contribute to a heavier midface read. HA filler placement varies by anatomy. Duration 9–12 months.

PDO thread lift

Submental Area

PDO thread lift can refine fullness under the chin that disrupts jaw definition. Additional submental contouring options discussed at consultation. Threads last 12–18 months.

Laser + microneedling + PRP

Skin & Hair

Laser hair removal addresses unwanted facial hair; microneedling, PRP, and laser resurfacing refine texture, pores, and acne scarring. Series individualized per patient at consultation.

Most patients begin with 2–4 corrective sessions staged over time, then transition to a maintenance schedule every 3–6 months. Staging lets Dr. Borakowski assess how each area responds and adjust the approach — helping create natural-looking results over time rather than reshaping the entire face in one sitting.

How a Feminization Session Works

  1. 1

    Consultation and facial assessment

Dr. Borakowski reviews your goals, anatomy, hormone-therapy status (if any), and which features you most want to soften. Together you map a staged plan — what to address first, what to layer in over time, and what may need a surgical referral.

  2. 2

    Topical anesthetic applied

Extra-strength topical numbing cream is applied 30–45 minutes before injection. Most HA fillers also contain lidocaine. Patients typically describe mild pressure or a brief pinch rather than sharp pain.

  3. 3

    Product and technique selection

Each zone is matched to the right tool — Botox for muscle-driven masculinization (masseter, frontalis, glabella), HA filler for volume and contour (cheeks, lips, chin), PDO threads for lift. Needle vs. cannula is chosen by anatomy and vascular safety, not preference.

  4. 4

    Precise placement

Filler is placed at the appropriate depth for each area. Botox doses are individualized — masseter dosing in particular is conservative on the first session, then titrated over follow-up visits as Dr. Borakowski sees how your muscle responds.

  5. 5

    Session complete & next steps

Most sessions take 30–90 minutes. Same-day return to normal activities is typical; PDO threads may need 1–2 weeks of activity modification. A follow-up review is scheduled to assess settling and plan the next staged session.

Dr. Natalya Borakowski, NMD brings 17 years of aesthetic medicine experience and a naturopathic medical degree from Southwest College of Naturopathic Medicine (BS Cell Biology and Genetics, ASU). Her naturopathic training shapes her approach to gender-affirming care: she considers overall wellness, skin changes, and aging concerns alongside injectable techniques.

Dr. Borakowski presented PDO thread techniques at the IMCAS World Congress in Paris (2025) and teaches aesthetic medicine at the University of Bridgeport. Patients on hormone therapy are encouraged to share their full medication list so treatment can be planned with full medical context in mind; any specific HRT considerations are addressed during the individual consultation.

Every facial feminization plan at Desert Bloom begins with a careful, individualized assessment — helping you understand what non-surgical techniques can achieve and what may need a surgical option. Because feminization often involves high-visibility, anatomically sensitive areas, treatment planning emphasizes careful attention to facial structure, product choice, and injection safety.

“There are definite structural differences between men and women — what makes each gender more attractive, or look more like that gender. The goal is to soften some features, like strong masseters, while adding gentle volume to others.”
— Dr. Natalya Borakowski, NMD

Frequently Asked Questions

What is the recovery time?
Most patients return to normal activities the same day. Mild swelling and bruising at injection sites typically resolve within 24 to 72 hours; cold compresses and arnica help. Masseter Botox has virtually no visible downtime. PDO threads may cause more noticeable swelling and bruising lasting one to two weeks, with activity modifications during healing.
How is non-surgical feminization different from surgical FFS?
Surgical FFS modifies bone — forehead, jaw, chin. Non-surgical feminization uses injectables to soften features, add volume, and reshape contours without incisions or general anesthesia. A non-surgical approach can create meaningful softening with far less downtime, but it does not replace bone-level surgery when that is the true anatomical need. The two can complement each other.
What areas make the biggest difference for feminization?
Clinical observation consistently places the brow and forehead region among the features with the strongest impact on gender perception. Cheeks, jaw, and lips are next priorities — volume and contour in these areas creates the soft, feminine proportions that matter most. Dr. Borakowski plans staging based on your anatomy and goals rather than a one-size-fits-all sequence.
How many sessions will I need?
Most patients begin with 2–4 corrective sessions, then transition to maintenance every 3–6 months. The number depends on how many areas are treated and the volume of product needed. A complete treatment plan is outlined during your assessment.
