What Is Non-Surgical Facial Feminization?

Non-surgical facial feminization — sometimes called liquid FFS — combines minimally invasive injectables, threads, and selected device-based treatments to soften the features that most strongly influence how gender is read in the face, without bone surgery. The goal is a natural, smooth shape: softer contours, rounder cheekbones, and a more feminine overall appearance.
The approach works on three levels. Dermal fillers add feminine volume and contour to cheeks, lips, and the areas around the mouth — when hyaluronic acid is used, results can be adjusted or dissolved with hyaluronidase if needed. Botox and other neurotoxins relax overactive muscles that contribute to a more masculine appearance, reduce frown lines, and smooth dynamic wrinkles. PDO threads lift and reposition soft tissue for a refined profile. Many patients also include laser hair removal or collagen-stimulating treatments as part of a broader plan to refine skin texture and reduce unwanted facial hair.

How a Feminization Session Works

Consultation and facial assessment
Dr. Borakowski reviews your goals, anatomy, hormone-therapy status (if any), and which features you most want to soften. Together you map a staged plan — what to address first, what to layer in over time, and what may need a surgical referral.

Topical anesthetic applied
Extra-strength topical numbing cream is applied 30–45 minutes before injection. Most HA fillers also contain lidocaine. Patients typically describe mild pressure or a brief pinch rather than sharp pain.

Product and technique selection
Each zone is matched to the right tool — Botox for muscle-driven masculinization (masseter, frontalis, glabella), HA filler for volume and contour (cheeks, lips, chin), PDO threads for lift. Needle vs. cannula is chosen by anatomy and vascular safety, not preference.

Precise placement
Filler is placed at the appropriate depth for each area. Botox doses are individualized — masseter dosing in particular is conservative on the first session, then titrated over follow-up visits as Dr. Borakowski sees how your muscle responds.
