What Causes Eye Bags?

Under-eye puffiness comes from two main sources: fat pad prominence and skin laxity. With age, weakening of the orbital septum allows lower-lid fat pads to bulge forward and create a persistent bag. Separately, lower eyelid skin loses collagen and elastin over time and begins to fold and sag, contributing to the baggy appearance even when fat herniation is minimal.

Genetics play a significant role — some people develop eye bags in their twenties while others don’t notice them until their fifties. Fluid retention, allergies, and sleep deprivation cause transient puffiness, but permanent structural bags don’t respond to lifestyle changes. Treatment Options for Eye Bags

Tear Trough Filler

When eye bags are driven by a sunken tear trough creating contrast with the fat pad — making the fat look more prominent than it actually is — tear trough filler corrects the contour at the lid-cheek junction. By blending the transition between lower eyelid and cheek, strategically placed filler reduces the visual impact of mild fat prominence without touching the fat itself. CO2 Laser

When the bag comes from loose, lax lower eyelid skin that folds and creases — particularly in patients with significant photoaging — skin tightening is the relevant intervention, not volume correction. Periorbital CO2 laser resurfacing contracts and thickens lower eyelid skin, providing visible firming and smoothing for the delicate eye area in appropriate candidates.
