A long-lasting filler that doesn’t need replacing every year

Most patients asking about Bellafill have already been through the cycle — temporary injectable fillers every six to twelve months, each time wondering if a more durable, non-surgical solution exists. There is one.

Bellafill is the only FDA-approved semi-permanent dermal filler. It combines PMMA microspheres with bovine collagen to deliver results lasting up to five years — not months. Important: the PMMA microspheres are a permanent implant. Bellafill cannot be reversed. That is what makes it different from every other filler, and why careful patient selection matters more here than anywhere else.

At Desert Bloom Skincare in Scottsdale, Dr. Natalya Borakowski, NMD offers complimentary Bellafill consultations. Her consultation-first philosophy means she will tell you honestly whether this is the right treatment for your goals — or whether another approach fits better.

Bellafill is one of several dermal filler treatments at Desert Bloom, alongside Sculptra, Radiesse, and lip augmentation.

What Is Bellafill?

Bellafill is a unique injectable dermal filler manufactured by Tiger Aesthetics Medical (formerly Suneva Medical). Its formula combines 20% PMMA (polymethyl methacrylate) microspheres suspended in 80% bovine collagen gel, with a small amount of lidocaine for patient comfort.

Unlike temporary fillers that dissolve within months, Bellafill works in two phases: the bovine collagen carrier provides immediate volume, while the PMMA microspheres act as a permanent scaffold — stimulating your body’s own collagen production over time to create durable improvement in your appearance.

Bellafill is FDA approved for two indications: nasolabial folds (since 2006, originally as ArteFill) and moderate-to-severe atrophic acne scars on the cheeks (since 2015). It remains the first and only filler with FDA approval for acne scars.

How Does Bellafill Work?

After Bellafill injections, the bovine collagen carrier absorbs naturally over one to three months. What remains are the PMMA microspheres — tiny, smooth, non-resorbable spheres your body cannot break down. Your body’s collagen-producing cells (fibroblasts) migrate to the microspheres and create a new collagen matrix around them, helping to lift treated areas and build a lasting structural foundation.

How Bellafill works — collagen microsphere scaffold and tissue integration mechanism

The result: your own collagen provides the structure — not a gel that dissolves. That is why Bellafill treatment delivers results lasting up to five years and why the Bellafill procedure produces smoothing of wrinkles and lines that holds over time.

What Does Bellafill Treat?

Bellafill treatment addresses both on-label and off-label concerns, helping you achieve durable, long-lasting results across a range of facial areas.

Important: Bellafill is NEVER used under the eyes (tear trough) or in the lips at Desert Bloom. Bellafill is not FDA-approved for lip augmentation, and the non-reversible nature of PMMA makes these targeted areas too high-risk.

Bellafill Treatment Areas

Bellafill is the only filler with FDA approval for acne scars. The acne scar clinical trial showed 64% of treated patients responded at six months versus 33% in the control group (FDA PMA P020012S009) — a clear distinction no other injectable filler holds.

Bellafill treatment areas — nasolabial folds cheeks and acne scars injection zones

Bellafill does not provide significant immediate volume like traditional HA fillers — it provides long-term collagen stimulation and structural support. For patients who have already tried temporary injectables and want long-lasting results that don’t need replacing, Bellafill can improve and maintain outcomes that other fillers cannot sustain.

How Long Does Bellafill Last?

Bellafill lasts up to five years — longer than any other injectable filler. The FDA post-approval study followed 1,008 patients across 23 centers and found an 87% retention rate at five years, with 83% patient satisfaction (Cohen et al., 2015). A separate review of 212 patients over four years confirmed consistent long-lasting results (Solomon et al., 2021).

Is Bellafill permanent? Here is the honest answer: the PMMA microspheres themselves persist indefinitely — they are non-resorbable. The aesthetic result evolves as natural aging continues, which is why “semi-permanent” is more accurate than “permanent.” But there is no reversing it. If a problem arises, surgical excision is the only removal option.

Some patients choose a touch-up on the same treated area approximately 12 months after the first Bellafill appointment, once the collagen carrier has fully absorbed and been replaced by natural collagen. This is not commonly needed — individual results vary.

For patients tired of scheduling filler injections every six to twelve months, Bellafill offers a fundamentally different timeline: long-lasting results that hold for years, not months.

Bellafill vs Sculptra vs Radiesse

Bellafill, Sculptra biostimulator, and Radiesse are all biostimulators — injectables that trigger your body’s collagen production rather than simply adding gel. The key differences are duration, mechanism, and whether results can be reversed.

Unlike hyaluronic acid fillers such as Restylane or RHA — which dissolve within six to eighteen months and can be reversed with hyaluronidase — Bellafill’s PMMA microspheres do not break down and cannot be dissolved. That permanence is a benefit for the right candidate and a reason for careful patient selection for everyone else.

Patients who currently use Botox or temporary fillers to address age-related lines often find that Bellafill provides a more stable structural foundation, reducing how often they need additional injections overall.

Bellafill Cost in Scottsdale

Bellafill cost is $800 per syringe (0.8 mL) in Scottsdale. A full box of 5 syringes is available for $3,500 when paid upfront — a $500 savings. Treatment cost depends on the targeted areas and depth of correction: acne scar treatment typically requires 1 syringe, while nasolabial folds usually need 2–3 syringes total.

Because Bellafill lasts up to five years, the Bellafill procedure can be more cost-effective over time than fillers requiring annual retreatment. We are happy to discuss cost comparisons during your consultation.

Is Bellafill Safe?

Skin Test Requirement
Bellafill contains bovine collagen. A skin test is required at least 4 weeks before your first Bellafill treatment — but only for patients who do not eat beef or have a history of bovine collagen sensitivity. Patients who regularly consume red meat are generally not at elevated risk for allergic reaction. If a skin test applies to you, we schedule it during your initial consultation visit.

Bellafill is FDA approved with over 650,000 syringes tracked in post-marketing surveillance from 2007 to 2017 (Hanke et al., 2019). Common side effects at the injection site include redness, swelling, and bruising — typically mild and resolving within 1–7 days. Some patients notice mild discomfort or itching that subsides within a few days. Most resume normal activities within 1–2 days after treatment.

The post-approval study reported a granuloma incidence of 1.7% over five years; broader surveillance data showed a rate of 0.002% per syringe. Granulomas can be difficult to treat. Dr. Borakowski follows strict injection protocols and monitors patients at follow-up appointments to catch and address any early signs.

Bellafill cannot be dissolved like HA fillers. If a problem arises, surgical excision is the only removal option. Rare but serious complications include granuloma formation and vascular occlusion — risks that require a provider with deep knowledge of facial anatomy and injection technique.

This is not a procedure to schedule on impulse. We will not rush you into it either.

Are You a Good Candidate for Bellafill?

Good Candidates for Bellafill

You may be a good candidate if you:

  • Are 21 or older with moderate to severe nasolabial folds, wrinkles, or acne scars
  • Are comfortable with semi-permanent, non-reversible results
  • Have passed bovine collagen skin test if applicable (required for patients who do not eat beef or have bovine collagen sensitivity — scheduled 4 weeks before treatment)
  • Are men or women of all skin types seeking long-lasting results without frequent touch-ups
  • Want a solution that reduces the need for additional injections year after year
Bellafill Is NOT Appropriate For:
  • Known bovine collagen allergy, history of anaphylaxis, or failed skin test
  • Active inflammatory acne (for acne scar treatment)
  • Tendency toward keloid or hypertrophic scarring
  • Pregnant or nursing
  • Immunosuppressed or on certain medications
  • Active infection at the treatment site

A good consultation does not always end with a procedure. Sometimes it ends with clarity — and a recommendation that a different option fits better. That is how we work at Desert Bloom.

Frequently asked questions

How long does Bellafill last? Up to five years, based on the FDA post-approval study of 1,008 patients (Cohen et al., 2015). The PMMA microspheres provide a permanent scaffold; the aesthetic result evolves with natural aging but the structural support remains. Individual results vary.
Is Bellafill FDA approved? Yes. Bellafill is FDA approved for nasolabial folds (2006, originally as ArteFill) and for moderate-to-severe acne scars on the cheeks (2015) — the only filler with approval for both indications.
Is Bellafill permanent? Semi-permanent. The PMMA microspheres do not dissolve, but the aesthetic result evolves as natural aging continues. Semi-permanent is more accurate than permanent — but the implant itself is not reversible.
Can Bellafill be reversed? No. Unlike HA fillers, Bellafill cannot be dissolved with enzymes. Surgical excision is the only removal option if a problem arises — which is why choosing an experienced provider matters more here than with any other filler.
Do I need a skin test? Only if you do not regularly eat beef or have a history of bovine collagen sensitivity. Patients who eat red meat regularly are generally not at elevated allergy risk. If a skin test applies to you, we schedule it at least 4 weeks before treatment during your consultation visit.
How much does Bellafill cost? $800 per syringe, or $3,500 for a box of 5 syringes paid upfront (a $500 savings). See the full treatment price list at /price-list/.
What are the side effects of Bellafill? Common side effects at the injection site include redness, swelling, and bruising — typically resolving within a few days. A rare but unique risk is granuloma formation (1.7% incidence in the five-year study).
How is Bellafill different from HA fillers like Restylane? Bellafill’s permanent PMMA scaffold provides long-term structural support for static lines (present at rest) and acne scars. HA fillers like Restylane and RHA are better suited for situations where a temporary filler is preferred, where flexibility matters, or where reversibility is important. HA fillers dissolve on their own and can be reversed with hyaluronidase; Bellafill cannot.
