Key facts about RF microneedling
RF microneedling is an FDA-cleared, minimally invasive treatment combining microneedling with radiofrequency energy to trigger collagen and elastin production in the dermis. The Virtue RF device delivers precise heat below the skin surface using insulated gold-tipped needles, with 3–5 days of social downtime and results that develop over 3 months.
Part of Desert Bloom’s laser and energy treatments platform, alongside CO2 laser resurfacing, erbium laser resurfacing, and Elluminate Mini.
What is RF microneedling?
RF microneedling uses insulated needles and radiofrequency energy to stimulate collagen production deep in the dermis. It tightens skin, smooths acne scars, and refines texture without the downtime of ablative lasers. At Desert Bloom Skin Care, Dr. Natalya Borakowski performs every treatment using the Virtue RF device, with settings individualized to your skin type and anatomy.
RF microneedling (also called radiofrequency microneedling or collagen induction therapy with RF) is a minimally invasive treatment that combines fine needles with radiofrequency energy delivered into the dermis. Gold-tipped insulated needles penetrate the skin surface while protecting the epidermis.
At the needle tips, RF energy heats tissue above 65°C, the temperature needed for collagen remodeling. This triggers neovascularization (new blood vessel growth), extracellular matrix regeneration (rebuilding the skin’s structural support), and new collagen and elastin synthesis. Skin becomes firmer and smoother, and the rejuvenated appearance develops gradually over several weeks.
The difference from traditional microneedling is thermal. Standard microneedling creates mechanical micro injuries only. Adding RF energy drives collagen production deeper and produces skin tightening that needles alone do not achieve. Because insulated needles concentrate heat below the epidermis, RF microneedling carries lower hyperpigmentation risk than ablative lasers for lighter skin tones.
How Does Radiofrequency Microneedling Work?
Dr. Borakowski evaluates skin concerns, skin type, and medical history. She assesses subcutaneous fat volume to determine safe depth and frequency settings based on your individual anatomy.
Topical numbing cream (BLT: benzocaine, lidocaine, tetracaine) applied for 45 minutes before treatment. Most patients feel pressure and warmth rather than pain.
The Virtue RF device passes over the treatment area for 30–45 minutes. Thirty-six insulated needles create micro injuries while radiofrequency waves deliver controlled heat into the dermis simultaneously.
RF energy heats the dermis, triggering the wound healing cascade. Support cell regeneration begins immediately. Collagen and elastin production continues for months, gradually restoring skin firmness.
A 15-minute calming mask is applied. Epionce Essential Recovery Kit (5-day) provided. Total appointment time: approximately 1 hour 45 minutes.
Skin Concerns RF Microneedling Can Treat
RF microneedling treats a range of skin concerns where deeper collagen remodeling and skin tightening is needed. The Virtue RF device and its three handpieces allow precise treatment across different areas and depths.
- Acne scars: rolling and boxcar scars respond best, and RF microneedling is effective as monotherapy for acne scarring. CO2 or erbium laser are more effective for deeper scarring.
- Fine lines and wrinkles: 20–60% improvement in skin laxity documented across studies
- Skin laxity and tighter skin: jawline, neck, jowling. RF energy stimulates collagen remodeling for firmer, tighter skin
- Large pores: 25–35% objective pore size reduction, and we love pairing RF with Erbium laser to deliver even better results
- Skin texture and tone: smoother, more even skin texture that helps rejuvenate dull or uneven skin
- Eye bags and periorbital aging: precision treatment with ExactRF handpiece
- Sun damage and stretch marks: body treatment with DeepRF handpiece
Am I a Good Candidate for RF Microneedling?
The ideal candidate has early signs of aging, acne scarring, or skin texture concerns and wants more dramatic results than traditional microneedling without extended downtime.
- Adults in good health with realistic expectations about results
- Mild-to-moderate acne scarring, fine lines, skin laxity, or texture concerns
- Fitzpatrick skin types I–III (RF microneedling is not performed on Fitzpatrick IV–V at Desert Bloom)
- Patients seeking a minimally invasive alternative to surgical repair or ablative laser
- Adequate subcutaneous fat in the treatment area
- Early signs of aging with desire for skin tightening without surgery
Use caution with: bleeding disorders or anticoagulants, recent dermal filler injections (RF microneedling should be performed BEFORE filler, not after, because of the risk of melting filler with RF energy; discuss timing with your provider), topical retinoids (discontinue five to seven days prior), autoimmune conditions.
Virtue RF: The RF Microneedling Device We Use
Desert Bloom uses the Virtue RF, an FDA-cleared RF microneedling device by Cartessa Aesthetics with 36 gold-tipped insulated needles, 31 robotic depth settings (0.1–3.5 mm), and three selectable frequencies (0.5, 1, 2 MHz). Sub-pulse technology distributes heat evenly for patient comfort.
Three handpieces allow precision across treatment areas: SmartRF (face/neck), DeepRF (body with cooling) and ExactRF (single needle for eyes and jawline). The depth of energy delivered is controlled by needle penetration depth, which can vary from 0.5–2.5 mm for the face. Dr. Borakowski assesses facial anatomy and fat volume before every treatment and selects appropriate depth settings based on individual factors. Patients with thin skin and minimal subcutaneous fat are not candidates for RF microneedling, and CO2 laser is a better option for them.
RF microneedling devices differ mainly in how deep the needles reach. Some reach up to 8 mm into the subcutaneous fat layer, which suits body contouring but carries a higher risk of facial fat loss. Virtue RF reaches up to 3.5 mm and is designed to stay in the dermis with precision depth control.
Does RF Microneedling Cause Fat Loss?
Fat loss is a documented risk, and it deserves a direct answer. On October 15, 2025, the FDA issued a Safety Communication reporting burns, scarring, subcutaneous fat loss, nerve damage, and disfigurement from RF microneedling devices. In the MAUDE database, 26 of 224 reported adverse events (11.6%) involved fat loss.
Fat loss happens when RF energy penetrates deep enough to damage fat cells. Risk is highest with lower-frequency settings, deep needle penetration on thin-skinned areas (periorbital, cheeks, temples), naturally thin faces, and repeated aggressive treatments.
The FDA is not banning devices. The communication emphasizes proper training and qualified providers. Complications like fat loss are not inevitable with appropriate technique, but we still take the concern seriously.

Who is a good candidate for RF microneedling?
RF microneedling at Desert Bloom suits healthy adults with Fitzpatrick skin types I to III who have mild to moderate acne scars, fine lines, skin laxity, large pores or uneven texture. You also need enough fat under the skin in the treatment area, and realistic expectations about gradual collagen results over about 3 months.
Desert Bloom does not perform RF microneedling on Fitzpatrick IV to V skin because the risk of hyperpigmentation is much higher. Standard microneedling is a safer option. Patients with thin skin and little facial fat are referred to CO2 laser resurfacing instead.
Do not book RF microneedling if you have:
- A pacemaker, defibrillator or other active electronic implant
- Pregnancy or breastfeeding
- An active infection or cold sore (herpes simplex) at the site
- Taken isotretinoin in the past 6 months
- A history of keloids, or gold implants in the area
Recent dermal filler, blood thinners, topical retinoids and autoimmune conditions need a timing discussion first. Stop topical retinoids 5 to 7 days before treatment.
What to expect during RF microneedling
An RF microneedling appointment at Desert Bloom lasts about 1 hour 45 minutes. Dr. Natalya Borakowski, NMD applies numbing cream for 45 minutes, treats your skin with the Virtue RF device for 30 to 45 minutes, and finishes with a 15-minute calming mask. Most patients feel pressure, warmth and a snapping sensation rather than pain.
- Consultation: Dr. Borakowski reviews your skin type and medical history and assesses facial fat to set safe needle depths, between 0.5 and 2.5 mm on the face.
- Numbing: a BLT cream (benzocaine, lidocaine, tetracaine) sits on the skin for 45 minutes.
- Treatment: 36 insulated gold-tipped needles deliver radiofrequency heat into the dermis as the handpiece passes over the area.
- Calming mask: 15 minutes, then you go home with a 5-day Epionce Essential Recovery Kit.
Expect redness, pinpoint bleeding and a sunburn-like feeling on the day. Skip makeup for at least 24 hours.
What results can you expect from RF microneedling?
RF Microneedling Before and After: What Results to Expect
Results develop gradually. At 4–6 weeks, skin texture improvement and firmness become noticeable. By 3 months, collagen remodeling peaks. Deep remodeling continues for several weeks and months after your final session. Studies document 20–60% laxity improvement and 25–35% pore reduction. Full results appear after the complete series plus the 3-month maturation window.
Most patients need 3–4 sessions spaced 4–6 weeks apart; severe acne scars may need 4–6. Maintenance of 1–2 treatments per year sustains collagen production and helps preserve the firmer, rejuvenated appearance.

What is recovery like after RF microneedling, and what are the risks?
RF Microneedling Recovery Day by Day
Plan for 3–5 days of social downtime. Many clinics advertise “no downtime.” We tell you the truth: budget 3–5 days.
Redness, pinpoint bleeding, moderate sunburn sensation. No makeup or active skincare. Some patients experience mild dryness or tightness, which is normal.
Redness and swelling peak. Skin feels tight. No retinoids, AHAs, BHAs, or vitamin C. Side effects at this stage are expected and typically resolve quickly.
Redness fades to pink. Light flaking normal. Most patients return to normal social settings by Day 3–4.
Skin visibly healed. Smoother appearance begins.
Firmness and skin texture improvement noticeable. Collagen deposition active. This is when most people start to see and feel meaningful improvements.
Full results, with collagen remodeling complete.
Avoid exercise 24–48 hours. Apply SPF 50+ sunscreen daily. Sun exposure must be avoided for two weeks. Makeup: avoid 24 hours minimum; mineral makeup safe at 48 hours.

“RF microneedling with Virtue RF is one of the most precisely controllable energy treatments available. Depth control, not frequency, is what keeps RF energy in the dermis and away from the fat layer. I set needle penetration depth based on each patient’s anatomy, skin thickness, and fat volume. Patients who are not candidates for RF microneedling are referred to CO2 laser instead. Every protocol is individualized based on anatomy, skin type, and treatment goals.”
