RF microneedling — also called radiofrequency microneedling or collagen induction therapy with RF — is a minimally invasive treatment that combines fine needles with radiofrequency energy to stimulate collagen and elastin production deep in the dermis.

Gold-tipped insulated needles penetrate the skin surface while helping concentrate RF heat below the epidermis, reducing unnecessary surface heat exposure when settings are selected appropriately. At the needle tips, RF energy creates controlled thermal injury in the dermis, triggering a wound-healing cascade that supports new collagen and elastin formation over time. The result is firmer, smoother skin that continues improving over several weeks and months.
The difference from traditional microneedling is thermal. Standard microneedling creates mechanical micro-injuries only. RF adds a second layer of thermal injury that drives collagen production deeper and produces skin tightening that needles alone cannot achieve. Insulated needles concentrate heat at the tip — below the epidermis — which is why RF microneedling carries lower hyperpigmentation risk than ablative lasers for lighter skin tones.
Dr. Borakowski assesses skin concerns, skin type, subcutaneous fat volume, and medical history. Depth settings are individualized to your anatomy.
Topical numbing cream (BLT: benzocaine, lidocaine, tetracaine) applied 45 minutes before treatment.
The Virtue RF device passes over the treatment area for 30–45 minutes. Thirty-six insulated needles create micro-injuries while delivering radiofrequency waves into the dermis simultaneously. Needle penetration depth — controlled by the cartridge setting — ranges from 0.5 to 3.5 mm depending on the treatment area and tissue thickness.
RF energy heats the dermis, stimulating fibroblast activity and collagen remodeling over the following weeks and months.
A 15-minute calming mask is applied post-procedure. Total appointment: approximately 1 hour 45 minutes.
RF microneedling treats a range of skin concerns where deeper collagen remodeling, skin tightening, and texture improvement are needed.
The ideal candidate has early signs of aging, acne scarring, or skin texture concerns and wants more dramatic results than traditional microneedling without the extended downtime of ablative lasers. Good candidates are in good health with realistic expectations, have adequate subcutaneous fat in the treatment area, and have Fitzpatrick skin types I–III.

A split-face trial found RF microneedling had milder side effects and shorter downtime than CO2 laser, but CO2 and erbium laser remain more effective for acne scarring.
Plan for 3–5 days of social downtime. Many clinics advertise “24-hour downtime.” In our experience, most patients should plan more realistically for 3–5 days of visible redness, swelling, or pinkness.
Redness, pinpoint bleeding at needle sites, moderate sunburn sensation. No makeup or active skincare. Avoid sun exposure immediately post-treatment.
Redness and swelling peak. Skin feels tight and sensitive. Discontinue retinoids, AHAs, BHAs, and vitamin C until healed.
Redness fades to pink. Light flaking is normal. Most patients return to normal social settings by Day 3–4. Side effects at this stage are expected and typically resolve quickly.
Skin visibly healed. Smoother appearance begins to emerge.
Firmness and skin texture improvement become noticeable. Collagen and elastin deposition active. Redness fully resolved.
Full results — collagen remodeling complete. Most patients see the most significant improvement at this stage.

Avoid exercise 24–48 hours post-procedure. Apply SPF 50+ sunscreen daily — mandatory sun protection for two weeks minimum.
This deserves a direct answer. On October 15, 2025, the FDA issued a Safety Communication reporting burns, scarring, subcutaneous fat loss, nerve damage, and disfigurement from RF microneedling devices. In the MAUDE database, 26 of 224 reported adverse events (11.6%) involved fat loss.
Fat loss occurs when RF energy penetrates deep enough to thermally injure fat cells. Risk is highest with deep needle penetration on thin-skinned areas — periorbital region, cheeks, temples — and in patients with naturally thin faces or minimal subcutaneous fat.
How we reduce risk: Energy depth is controlled by needle cartridge penetration depth, not frequency. Dr. Borakowski assesses facial anatomy and fat volume before every treatment and selects needle depth based on individual factors. Patients with thin skin and minimal subcutaneous fat may not be appropriate candidates for RF microneedling. In those cases, Dr. Borakowski may recommend another resurfacing or collagen-stimulating option, such as CO2 laser, depending on skin type and goals. Virtue RF’s 31-level depth control allows precision to the tenth of a millimeter.
The FDA is not banning devices — the communication emphasizes proper training and qualified providers. Complications like fat loss are not inevitable with appropriate technique. We take these risks seriously and assess every patient before treatment.
